CONSENT TO GENERAL TREATMENT AND TO FIRST AID
I/We, [name below] give consent to my/our child to receive first aid services provided by the school nurses.
He/she may be given non-prescribed medicines to treat minor illness or injury such as:
- Syrup Paracetamol, Syrup Buscopan, Duscopan Tablet, Syrup Piriton / Loratadine, Syrup Iburofen, ORS [Oral Rehydration Solution], Paracetamol Tablet, Olbas oil [Inhalant Decongestant], Throat Lozenges
EMERGENCY MEDICAL TREATMENT / CONSENT FOR HOSPITAL TREATMENT
We will always contact you if your child suffers anything more than a trivial injury, or if he or she becomes unwell during the school day, or should we have any worry or concern about his/her health. A medical/injury report will accompany your child home to notify you of any treatment given in the school. We will ask you to collect your child if he/she becomes ill during the school hours, but in an event that a child requires the service of a doctor or hospital treatment, which cannot be handled in the school clinic, LPSS is in partnership with Lagoon Hospital Bourdillon, Ikoyi. The partnership is put in place to take care of emergencies. This is not to take the place of parent's choice of hospitals, as it is mainly for emergencies that cannot be handled in the school clinic. We shall be authorized to make the decision on your behalf should consent be required for urgent treatment recommendation by a doctor, (including anesthetic or operation). Rest assured that you will be contacted if your child is being taken for such treatment, and please note that such treatment will be paid for by the parent.
